Tessa's background is somewhat unknown.Though it's apparent that she is highly experienced and has been doing her job for quite a long time.When Dean was having an out of body experience, as a result of his coma, Tessa originally appeared as a confused spirit, whose body is dying from a surgical error.After Dean finds out that a Reaper is after him, Tessa later meets up with Dean, dressed in an entirely different outfit than her hospital clothes.She revealed her true identity of being the Reaper in the hospital and the reason for her deception was to try to get Dean more conformable, so that she could talk with him.She wanted to help him move on, though Dean was reluctant to do so.He wanted to fight for his life and not leave his father and little brother behind.She proceeded to try to convince him that it was for the better that he agree to move on to the other side and that if he remained a earth bound spirit, he would eventually be driven into madness.She was later possessed by the demon Azazel, after John Winchester made a deal for his son to be brought back to life, in exchange for his soul.A possessed Tessa, sent Dean's spirit back into his body.Dean's memory of his encounter with Tessa, was wiped from his mind, until they would eventually meet again.
